Enter the case totals from your OSHA 300A summary and the hours worked. You get the TRIR and DART rates that prequalification forms ask for.

## Where the numbers come from

Columns G to J of the 300A count cases by their most serious outcome: deaths, days away, job transfer or restriction, and other recordable cases. Together they are your recordable cases. H plus I are your DART cases. The summary must be posted from February 1 to April 30 of the following year.

### Do I have to fill out a 300A if there were no injuries?

Yes. Employers who must keep OSHA records post the 300A every year, even with zeros (29 CFR 1904.32).
### Who signs the 300A?

A company executive: an owner, officer, the highest-ranking official at the establishment, or that person's supervisor.
### Do I have to submit it online?

Establishments in construction with 20 to 249 employees, and any establishment with 250 or more, submit the 300A to OSHA's Injury Tracking Application by March 2 (29 CFR 1904.41).
