This is the full template. Words in [brackets] are what you fill in for your company; everything else is the fixed wording, checked against the rule cited under each section.

Cal. Code Regs. tit. 8, § 3203

Cal. Code Regs. tit. 8, § 3203

6. Hazard correction

Unsafe conditions and practices are corrected in a timely manner based on how severe they are. When an imminent hazard cannot be corrected right away without endangering anyone, all exposed employees are removed from the area except those needed to correct it, who are given the protection they need.

Cal. Code Regs. tit. 8, § 3203

7. Training

Training is provided:

  • When this program is first established.
  • To all new employees, and to employees given a new job assignment they have not been trained for.
  • When new substances, processes, procedures or equipment introduce a new hazard.
  • Whenever the company learns of a new or previously unrecognized hazard.
  • Supervisors are trained on the safety and health hazards their crews may be exposed to.

Cal. Code Regs. tit. 8, § 3203

8. Construction requirements: Code of Safe Practices and safety meetings

The company's written Code of Safe Practices is posted at each job site office or given to each supervisor. Supervisors hold toolbox or tailgate safety meetings with their crews at least every 10 working days, and supervisory safety meetings are held periodically.

Core rules of the Code of Safe Practices:

  • Report every injury, however minor, to your supervisor right away.
  • Wear a hard hat, safety glasses and the other PPE your task requires; inspect it before use.
  • Use fall protection where required; never remove a guardrail or cover without permission.
  • Inspect ladders, scaffolds, tools and cords before use; tag out and report damaged equipment.
  • Never work under the influence of alcohol or drugs, and no horseplay on the job.
  • Keep work areas, walkways and exits clear; store materials safely.
  • Only trained and authorized workers operate equipment or work on energized circuits.
  • Know where the first aid kit, fire extinguisher and emergency phone numbers are on each site.
  • Trade-specific rules: [Safe work rules specific to your trade]

Cal. Code Regs. tit. 8, § 1509

9. Recordkeeping and access

Inspection records — who inspected, the unsafe conditions found, and the action taken — and training records — employee name or ID, dates, type of training and trainer — are kept for at least one year. Employers with fewer than 10 employees may keep inspection records only until the hazard is corrected, and may keep a log of instructions given instead of individual training records.

Employees and their representatives may see and get a free copy of this program within 5 business days of asking.
