Is a flare-up of an injury already on the log a new OSHA recordable case?
No new entry. If the worker had not recovered completely and no new workplace event caused it, it is the same case: update the existing entry, for example with new days away or restriction. A new event, or a recurrence after full recovery, is a new case.
Why
- It continues a case already on the log, so you update that entry instead of adding a new one. 29 CFR 1904.6
This answer comes from the same rule-by-rule check used in the HardHat Binder OSHA log, based on the facts described above. Different facts can change the result.

Answer a few questions for each injury and the log tells you whether it's recordable, which column it goes in, and fills the 300, 300A and 301 forms.
