How is a work-related death recorded on the OSHA 300 log?
A work-related fatality is always recordable and is entered in column G, the most serious outcome. The fatality must also be reported to OSHA within 8 hours of learning about it.
Report to OSHA within 8 hours: call 1-800-321-6742, use the online form, or contact the nearest area office (29 CFR 1904.39).
Why
- The case resulted in death. Also report the fatality to OSHA within 8 hours. 29 CFR 1904.7(b)(2)
